ADOPTION AGREEMENT
Alley's Feathered Palace & More assumes no responsibility for the health, disposition or medical condition
of any adopted parrot. It is strongly recommended that all birds have a complete new bird
exam at an avian vet within the next two days. If you own another bird, please quarantine
the adopted bird from your other birds, for health reasons, for 30 to 60 days. All medical
costs incurred are the sole responsibility of the adoptor.
Donor and adoptor may make their own arrangements regarding the adoptee, pertaining to
visitation, phone contact and exchanging photographs, if the parties involved so desire.
The donor relinquishes all rights of ownership of said parrot and/or accessories
The undersigned agree to all terms set forth in the above agreement.
